Visit Structure:

Initial in-office appointment with intake form completion with diagnosis specific testing tools, complete history & physical, membership selection recommendation, discussion of additional services available, as well as medication evaluation and treatment. LAB order provided at visit with follow-up via telehealth or in-office at 1 mo.

Follow-up visits are utilized to review your labs, follow-up on medication regimens, and discuss further plan of care.
These may be conducted as standalone visits or as part of our membership plan.
